Data-Driven Decision Making - We support our dual approach to grantmaking that includes both a pro-active, strategic initiative that is data driven, and a responsive, unsolicited grantmaking approach that trusts the community to bring pressing needs to our attention.
Building the Organizational Capabilities and Capacities of our Nonprofit Partners- We value building the organizational capabilities and capacities of the nonprofits we fund and have identified capacity building as one of our priorities in our strategic planning process.
Geographic Service Area - We value serving our entire geographic service area; however, the "hub" of our work remains in the District and we continue to focus our work in areas and communities where there are the greatest racial and ethnic disparities in health outcomes, particularly the District and Prince George’s County, Maryland.
Serving Diverse Communities - We value serving diverse communities, particularly those where racial and ethnic disparities in health outcomes are the greatest, the most deep-rooted, and where there are large numbers of people who have been traditionally excluded and isolated from the health care system.
Attacking Health Problems Strategically - We emphasize attacking health problems on multiple fronts that affirm our community-based, consumer-focused, general public health, and systems change approach in this regard.
Supporting Experimental Programs - We value supporting experimental (innovative, new, and emerging) programs that might not normally receive foundation support. Consequently, our dollars have become seed funding for projects that were able to demonstrate certain outcomes and attract additional funds.
Funding Change Agents - We value funding organizations that have the capacity to make fundamental changes that ultimately impact community health.
Outcome-Driven Programming - We value outcome-driven programming and affirm our current work in this area. We assure that our grantees are clear on what they are going to measure, but recognize that outcomes for some developmental programs may not be achievable within the term of the grant.
Documented Measures of Success - We value having well-researched, documented measures of our success.
Grant Making Role - We value our important and principal roleas community grantmaker and are confident with the current balance interms of our multiple roles. However, we support examining more pro-active roles in terms of community information sharing and as an educated risk-taker.
Consumer Input - We highly value consumer and community input in our strategic planning process.
Catalytic Role - We value being a catalyst for stimulating interest in the community and view ourselves as a catalyst for change.
Our Goals
CHF pursues four goals:
We make grants to groups, organizations, and agencies for projects that address our priorities and promote our values. Our Strategic Initiative grants are awarded through a formal process in which we solicit grant proposals each spring. We also make Unsolicited grants throughout the year in response to pressing needs in the community that cannot be anticipated.
We help to improve the capacity of our grantees by offering or sponsoring special training and technical assistance.
We continue to support critical partnerships and build key alliances within the community that advance our mission, priorities and values, taking advantage of our role as convener and catalyst.
We utilize a communications plan that informs our community about our work, our nonprofits partners, and new developments in community health. As part of this plan, we contribute to public policy development by communicating with policymakers and providing objective information
